The Memory Care Coordinator plans, directs, and assists in all cottage care staffing and service needs. The ideal candidate must have the ability to manage team processes, promote a team environment, have knowledge of medication and administration procedures, as well as the social, psychological, and recreational needs of Alzheimer’s and memory impaired residents.

JOB RESPONSIBILITIES:

    Manage
  • Recruit and hire cottage care staff.
  • Manage and direct cottage care staff.
  • Complete scheduling for cottage care staff.
  • Orient, monitor progress, and reinforce training of cottage care staff.
  • Assist the Resident Care Coordinator and Executive Director in implementing state regulations, as required.
  • Complete all HHHunt and state-required documentation in a timely manner.
  • Function as part of the management team (manager on duty coverage, presenting the community, planning and participating in staff meetings, planning and implementing marketing and community events).
    Resident Care Responsibilities
  • Develop a care team and residential living environment that supports overall Cottage Care philosophy.
  • Create and coordinate implementation of ongoing person-centered activities and lifeskills programming.
  • Complete resident assessments in a timely manner as required by HHHunt and state regulations.
  • Coordinate resident care service needs in collaboration with care team and community department heads, as appropriate.
  • Monitor physician orders to assure follow-through and completion.
  • Monitor all aspects of medication administration.
  • Audit Medication Administration Records (MAR) and notify pharmacy of changes.
  • Consult with families and/or physicians to coordinate resident care needs.
  • Communicate with physicians and other healthcare professionals regarding medications, treatments, and/or therapies.

SUPERVISORY RESPONSIBILITIES:

Recruit, train, supervise, evaluate performance for—and delegate responsibilities to—cottage care staff.

KNOWLEDGE AND SKILLS:

  • Experience in and knowledge of diagnoses.
  • Knowledge of medication and administration procedures.
  • Knowledge of resources for seniors and of related agencies.
  • Understanding of the social, psychological, and recreational needs of Alzheimer’s and memory impaired residents.
  • Ability to exercise confidentiality following HIPAA guidelines.

  • Must be pleasant, friendly, and helpful at all times, ensuring proper and timely service.

QUALIFICATIONS:

  • Must be 21 years of age or older.
  • Experience or training in an equivalent setting preferred.
  • RN/LPN or Bachelor’s degree preferred.
  • MedTech certification required if not LPN/RN.
  • General understanding of, and concern for, the needs of seniors.
  • Aptitude and previous experience with Alzheimer’s and memory-impaired residents.
  • Ability to work in an environment conducive to caring for residents without posing a substantial safety or health threat to self or others.
  • Ability to manage team processes and promote a team environment.

Qualifications

  • Assists Director of Sales and Marketing, when requested, to coordinate needs relating to logistics of new resident moves, including such things as transference of keys, furniture move-in coordination, unit preparation, new resident orientation and welcoming, etc.
  • May provide input regarding the development of promotional brochures, videos, and mailings used in networking referrals and marketing to prospective residents.
  • Prepares written reports and communicates status of current success in a clear and concise manner.
